On many levels there are enemies to be shot, by holding the joystick forward. At the end of certain levels are large enemy dragons, inspired by the game Space Harrier, which must be destroyed before the time runs out. Other hazards include large yellow walls with doors that open and close. A timer ticks down constantly, and only stops when the player reaches the grey tiles that mark the end of a level. Remaining time is carried over to the next level, and a bonus score awarded based on the number of seconds left. (The timer is reset after each dragon level). There are 24 levels in total. At the end of the game a rating is awarded, from Hilarious to Superblazer for completing the game.

Cosmic Carnage is a one-on-one fighting game set in outer space. The story begins as a prison barge is hijacked by it's cargo in an escape attempt. The resulting chaos caused the barge to crash into a military cruiser. The impact caused extensive damage to both ships. There were only 8 survivors, 4 from each ship. What's worse, there's only one escape pod. You control one of the 8 survivors, and have to finish off all the others to escape with your life.

In the first stage, the player must fend off meteor showers from all four sides of the screen by pushing the joystick to fire in the desired direction. The second stage requires the player to pilot a shuttle to a planet and use its tractor beam to pick up life forms. While near the planet's surface, planetary defenses will fire at the shuttle. If hit, one previously captured specimen will be freed, forcing the player to retrieve another. After a set period of time, a klaxon will warn of renewed meteor activity, and the player must return immediately to defend the ark.
