Felton Pinkerton wanted to take his wife to the fair, so he didn't get a chance to spray pesticide on his crop of corn. But due to his laziness, all of the bugs have now arrived and are trying to eat every last bit of corn! You control Felton who is armed with a spray gun. Your crop of corn is displayed on the left side of the screen, and you need to protect it from the incoming hoard of inchworms, moths, caterpillars, slugs, and other assorted bugs which approach rapidly from the right. If any of the bugs sneak past you, they will eat part of your corn crop. The game ends when all of the corn has been eaten. On each level you need to spray all of the bugs that appear in order to move on to the next round; as the rounds progress, the bugs become faster, more numerous, and home in quicker on the corn.

Worms is a turn-based strategy game. It features up to 4 teams of 4 worms, aiming to destroy the others on a generated terrain. Each worm has 100 hit points, and dies when his hit points fall to 0. Upon death, a worm explodes, causing damage to everyone around. Gameplay is turn-based. Each turn, the player can control one specific worm from his team. The worm can crawl left and right or jump. However, there is a time limit to make a move; also, if the worm falls from a great height, it loses health and the turn ends immediately; and if a worm falls into water or offscreen, it dies. Each turn, a worm can also make a single attack: the player can aim up and down, choose a weapon and then fire it. After attacking, the turn ends.

The gameplay itself hasn't changed much from its predecessors, with the exception of the three-dimensional view, which allows the player more freedom, and more possibilities, to complete the task at hand. Modes like Campaign and Quick Match return from previous versions. The game also features a multiplayer feature, as well as the ability to edit and create teams. The objective of most of the matches is to eliminate the opposing forces' worms, whereas the Campaign mode gives the player sets of specific goals which he needs to complete. The campaign consists of 35 small missions in which the player has to utilise their worms to complete a certain task, like destroy enemy worms, collect a certain crate, or even unique missions, like having to detonate 16 hidden landmines in a certain time. All of the missions give awards depending on how well you do. Gold medals usually unlock bonuses as for example maps, challenge missions, infos about weapons or voice banks. In the challenge missions in you have to use a weapon/utility to collect targets that add to your timebank which increases steadily. Getting a gold medal here unlocks maps or locked weapons.

Worms 4: Mayhem has a lot more in-depth story mode than the previous games. Instead of just completing missions with some minor background story told, proper introductory cut-scenes reveal information about the mission's background. The story actually begins in the tutorials, in which the player's own team of worms arrives to the Worminkle University. There they meet Professor Worminkle (who resembles Albert Einstein and Doc Brown), who trains the player and teaches the methods of using various weapons. In Story Mode, Worminkle sends the player to destroy the enemies buildings and sneak into their construction sites.
