As Albert, the player will be part of a team of medical scientists in charge of monitoring the fugitives' mental stability. If Albert does not confront his greatest weaknesses and deepest fears, he will not survive the claustrophobic depths of the underground from which there is no escape and where the darkness hides more secrets than simply the growing madness of its inhabitants. Afterfall: InSanity is a survival-horror action game set in a post-apocalyptic world, the Afterfall universe. We are presented with an alternate timeline: Nazi Germany managed to create a nuclear bomb in early 1945, thus ending the war and forcing the allied forces into negotiations. A long period of cold war followed. Eventually, Germany detonated an experimental fusion bomb in 2012, starting World War III. The resulting nuclear mayhem forced survivors to hide in underground shelters. Albert can wield several kinds of melee weapons as well as firearms, but ammunition for the latter is scarce. Thus, hand-to-hand combat is important. He also has a flash light available. Strenuous actions like sprinting or melee attacks are limited in duration by the player's stamina, which is indicated by a corresponding bar on the HUD. There is also a mechanism called FearLock: If Albert's fear level rises beyond a certain threshold, FearLock kicks in and influences the way he performs attacks.

The game is played from behind your Navy F-14 Tomcat as you blast oncoming fighter planes trying to bring you down. Perform complete barrel rolls by holding left or right as your machine guns continuously fire at anything in their path. Speed up by activating your afterburners or slow down to get a lock on your target. You also have a limited number of AIM-54 Phoenix missiles on board to use as you see fit, which can be replenished after completing a series of stages (a plane will fly overhead and automatically load them onto your F-14 Tomcat).

A branching storyline and time limits force gamers into split-second decisions, but once the "Climax Gauge" is full, time slows down momentarily to let pilots unleash a screen-clearing fusillade of bullets.
