Clock Tower II: The Struggle Within, known in Japan as Clock Tower Ghost Head (クロックタワーゴーストヘッド?), is a point-and-click adventure game created by Human Entertainment, published by Agetec, and released for the PlayStation in 1998. It is a spin-off to the Clock Tower series and has almost nothing to do with the previous games besides its gameplay. Following its predecessors, the gameplay is more about hiding and solving puzzles, rather than fighting the player's pursuers. This degenerates as the plot advances, and the main character is eventually forced to use guns and other tools in several endgame action sequences. Its interface consists of a point-and-click system similar to the one used by Sierra Entertainment games, such as the Gabriel Knight series. There is an inventory screen that appears if the user drags the pointer to the upper side of the screen. The game's engine looks similar to Clock Tower 2 (Clock Tower in North America), with a few minor alterations.

On each of Clockwiser's levels, you are presented with a level layout, and a target layout, with the aim being to transform the former to match the target. The levels consist of a collection of cogs, which can be rotated either clockwise or anti-clockwise to achieve this goal. There are lots of special blocks to help out, including bombs, diamond-producers and anti-gravity blocks. Each level has its own time limit, ranging from seconds to minutes - this doesn't start until you make your first move, and you can look around the level at your leisure before making a stab at it. Each level has its own password, which makes progression a little easier.

Sir Tongara de Pepperouchau III ("Pepper" for short) is a toy soldier. He is in love with the Clockwork Fairy Princess, Chelsea, whose voice wakes up the toys of the house every night at midnight. But he is clumsy and something of a laughingstock, especially when compared to his friendly rival Ginger who is also after Chelsea's heart. One night Chelsea is stolen away by an unknown force, which also hypnotizes some of the lesser toys to become fierce minions and stand in the way of anyone who would try to rescue her. If there's no voice to wake them up anymore then the toys will never live again, so Pepper and Ginger head off to find Chelsea before it's too late.

Clockwork Knight 2 uses identical gameplay to that of its predecessor, right down to using all the same items and having four rooms with three levels each, plus a final boss. However, there are three minor additions: Four playing cards are scattered around each level. Spinning all four cards gives players a Gold Key; collecting all 32 cards in the game yields a secret code. There are some forced scrolling levels in which Pepper rides on the back of his steed Barobaro. Attacking is done not with Pepper's key in these levels, but by firing Barobaro's head at enemies. An additional game mode called "Bosses Galore" lets the player control either Pepper or Ginger in fighting all the bosses of both Clockwork Knight games one after the other. Doing well in this mode is said to unlock a mini-game.
