During a severe lightning storm, the security systems at the Dixmor Asylum for the Criminally Insane go offline, allowing inmates Daniel Lamb and Leo Kasper to escape. Daniel is disoriented and amnesiac, unable to remember anything about himself. Under Leo's guidance, he makes his way back to the ruins of his old home, which is located nearby. Inside, Daniel recovers drugs which help to clear his head, and discovers hints about the details of his former life. He and Leo follow a series of clues to unravel the secrets of Daniel's past, killing anyone who interferes; along the way, they are pursued by bounty hunters and agents of a mysterious organization called "the Project".

The game takes place in the mansion of the Edison family: Dr. Fred, Nurse Edna, and their son Weird Ed. Living with the Edisons are two large, disembodied tentacles – one purple and the other green. The player starts out by selecting two of six characters to accompany protagonist Dave Miller. The intro sequence shows that a meteor crashed near the mansion 20 years earlier. The sentient meteor took control of the family and caused Dr. Fred to begin conducting human experiments while plotting to take over the world. After the introduction, Dave and his two companions prepare to enter the mansion to rescue his cheerleader girlfriend, Sandy Pantz, whom Dr. Fred has kidnapped.

In each of the twenty caverns are several flashing objects, which the player must collect before Willy's oxygen supply runs out. Once the player has collected the objects in one cavern, they must then go to the now-flashing portal, which will take them to the next cavern. The player must avoid enemies (listed in the cassette inlay) as "...Poisonous Pansies, Spiders, Slime, and Manic Mining Robots...", which move backwards and forwards along a predefined length at constant speeds. Willy can also be killed by falling too far, so players must time the precision of jumps and other movements to prevent such falls or collisions with the enemies.

Mansion of Hidden Souls is an adventure game created entirely with full-motion video. It takes place in a pre-rendered mansion that the player explores from a first-person perspective. As they wander from room to room, the action unfolds via video sequences. The player takes on the role of Jonathan, a boy whose sister has wandered into the spooky mansion. When she entered the mansion, she disappeared! What's more, the mansion is full of talking butterflies. The player must search the whole mansion to unlock its secrets and find Jonathan's sister.
